Abstract: To determine if polygraphic variables (PV) are associated with optic nerve dysfunction (OND) in obstructive sleep apnoea-hypopnoea syndrome (OSAHS).Prospective cohort of OSAHS with CPAP. Patients undertook an initial optical coherence tomography (OCT) to measure their baseline Bruch?s membrane opening-minimum rim width (BMO-MRW).OCTs were repeated during the follow-up. Demographic and anthropometric variables were recorded and cardiovascular comorbidities, Epworth Sleepiness Scale(ESS)score and the PV:AHI,cumulative time percentage with oxygen saturation<90%(CT90) and SpO2.Logistic regression analysis was carried out with BMO-MRW as the dependent variable and AHI and nocturnal respiratory failure (NRF) as the independent variables, where NRF was:CT90?30 orSpO2?90%.36 patients (58.3% male,59.42years,mean BMI34.39).The most common comorbidity was hypertension (61%),followed by dyslipidaemia (44%),DM (25%).The mean AHI was 46.30± 9.08, mean CT90 10.25±5.97 and mean SpO292%. Mean ESSscore 9.83±3.94.Univariate analysis revealed NRF to be the sole predictive factor of OND in OSAHS (p=0.049), with a relative risk almost three times higher compared to controls (RR=2.78). However,multivariate analysis did not show NRF to have a statistically significant association (p=0.066)with OND, despite an even higher RR of 5.6.AHI didn´t demonstrate any association with OND(p=0.448,RR=0.99).The only statistically significant predictive factor from multivariate analysis was DM(p=0.023, RR=5.6).NRF may be the main polysgraphic variable linked to OND in OSAHS. Patients with NRF have approximately 5.6 times higher risk,with a p-value very close to statistical significance.This study was limited by the very small sample size.
