Abstract: CD30-positive primary cutaneous lymphopro-liferative disorders include several entities with differ-ing clinical pres entation but overlapping histologicalfeatures, including lymphomatoid papulosis and pri-mary cutaneous anaplastic large cell lym phoma(C-ALCL). DUSP22–IRF4 locus translocation is pres-ent in 20–57% of C-ALCLs, and has also beendescribed in a series of 11 lymphomatoid papulosispatients, where it was associated with a particularbiphasic histological pattern, including pagetoid reti-culosis-type epidermal in?ltration. We aimed to studywhether the presence of this translocation may de?nedistinctive histological features in C-ALCL.Methods and results: We collect ed three cases ofC-ALCL with histological features similar to those described in the new variant of lymphomatoid papulosiswith 6p25.3 rearrangement. We studied their histo-logical features and immunophenotype, using a panelof antibodies against CD30, TCR-bF1, TCR-c, CD4,CD8, CD20, Ki-67 and ALK. FISH analyses were per-formed using an IRF4–DUSP22 break-apart probe forthe study of the 6p25.3 rearrangement. FISH resultswere positive in the three cases, which all showeddistinctive histological and immunohistochemical fea-tures: a diffuse dermal in?ltrate of atyp ical medium-to-large cells, and marked epidermotrophism withsmall, atypical intra-epidermal lymphocytes.Conclusions : Ou r ?nd ings suggest that the presenceof 6p25.3 rearrangement might be related to this par-ticular biphasic pattern.
