Abstract: Introduction: The female artificial urinary sphincter is a device indicated in cases of persistent or recurrent stress urinary incontinence after anti-incontinence surgery or in neurogenic patients. It has a special indication in patients with stress urinary incontinence and detrusor hypocontractibility in the emptying.
The laparoscopic and robotic approach has facilitated the implementation of this technique. The posterior approach to the bladder neck by dissection the bladder-vaginal space allows a direct vision of it, reducing the risk of urethral injury during the surgery.
Material and methods: We present the clinical case of a 61-year-old woman with an history of anterior vaginal plasty and TOT, requiring subsequent tape removal due to vaginal pain. After reoperation, she presented severe stress urinary incontinence, and in the complementary tests we observed an incompetence of the urethal sphincter. Laparoscopic implantation of the AMS 800 artificial urinary sphincter was decided. The steps of the procedure and the results of the surgery are described in this video.
Results: Surgical time was 130 minutes. The postoperative period was uneventful, and the patient was discharged 24h hours postoperatively. The bladder catheter was removed 5 days after the procedure, and the artificial sphincter was activated 8 weeks after the procedure. Three months after surgery, the patient is continent, presenting only urine loss after urination, until the cuff is completely filled, which has meant a great improvement in her quality of life, reported in validated questionnaires (ICIQ-SF score 2/10; PGI score 2/7).
Conclusion: The implantation of a female artificial urinary sphincter is a good treatment option in selected cases of stress urinary incontinence, with high patient s atisfaction and improvement in their quality of life.
