Abstract: Abstract: Objective: Polymyalgiarheumatica(PMR)isoftenthepresenting manifestationofgiantcellarteritis(GCA). Fluorine-18-fluorodeoxyglucosepositronemissiontomography/computedtomography(PET/CT)scanoften disclosesthepresenceoflargevesselvasculitis(LVV)inPMRpatients.Weaimedtoidentifypredictivefactorsof a positivePET/CTscanforLVVinpatientsclassifiedashavingisolatedPMRaccordingtowell-establishedcriteria. Methods: A setofconsecutivepatientswithPMRfromasinglehospitalwereassessed.Allofthemunderwent PET/CT scanbetweenJanuary2010andFebruary2018based onclinicalconsiderations.PatientswithPMR associatedtootherdiseases,includingthosewithcranialfeaturesofGCA,wereexcluded.Theremaining patients werecategorizedinclassicPMR(iffulfilledthe2012EULAR/ACRclassification criteriaatdisease diagnosis; n ¼ 84) oratypicalPMR(whodidnotfulfill thesecriteria; n ¼ 16).Onlyinformationonpatients withclassicPMRwasassessed. Results: Themeanageofthe84patients(51women)withclassicPMRwas71.4 7 9.2 years.APET/CTscanwas positivein51(60.7%).PersistenceofclassicPMRsymptomswasthemostcommonreasontoperformaPET/CT scan.Nevertheless,patientswithpositivePET/CTscanoftenhadunusualsymptoms.Thebestsetofpredictorsof a positivePET/CTscanwerebilateraldiffuselowerlimbpain(OR ¼ 8.8,95%CI:1.7?46.3; p ¼ 0.01),pelvicgirdle pain(OR ¼ 4.9,95%CI:1.50?16.53; p ¼ 0.01)andinflammatorylowbackpain(OR ¼ 4.7,95%CI:1.03?21.5; p ¼ 0.04).
